A new quality improvement initiative has been added to the PHASES program to strengthen patient care through focused mini audits and audit activities.
Developed to support continuous improvement in clinical practice, the Mini Audit and Audit Activity provides a simple, structured approach for practices to review their data, identify opportunities for improvement and track progress over time.
The tool encourages teams to reflect on key areas of care, including chronic disease management, preventive health and medication safety, by analysing practice data, discussing findings and implementing small, achievable changes.
It also supports practices to break down larger goals into manageable steps, helping to find new ways to enhance patient outcomes and strengthen evidence-based care.
The activity supports accreditation requirements and aligns with the Quality Improvement (QI) PIP framework, helping practices demonstrate their commitment to ongoing improvement and patient safety.
