PHASES and HealthPathways
HealthPathways supports PHASES by providing evidence based, locally agreed guidance for cardiovascular disease prevention and management. This includes clinical pathways and referral information to support consistent, high-quality care across Queensland.
Pathways are reviewed regularly by GP clinical editors, with input from local specialists and subject matter experts, so guidance stays current and trusted.
Access your local PHN HealthPathways portal below. We recommend adding it as a shortcut on shared practice devices so clinicians can open it quickly during consultations.

About PHASES
Purpose of PHASES
PHASES (Preventing Heart Attacks and Stroke Events through Surveillance) is a Queensland primary care initiative focused on reducing the burden of cardiovascular disease. It supports primary care teams to identify people at highest risk of heart attack and stroke earlier, so risk factors can be managed sooner and more effectively.
How Primary Sense supports PHASES
Primary Sense™ provides real-time insights from practice data for practices using Best Practice or Medical Director. These insights support opportunistic identification during consultations and systematic recall activities to guide targeted cardiovascular risk management.
Who can participate
Participation is open to primary care providers across Queensland, including general practices, allied health providers, Royal Flying Doctor Service, Aboriginal Medical Services and Aboriginal Community Controlled Health Organisations and other relevant services.
Cardiology specialists who see referred patients at risk are also encouraged to take part. Participation is voluntary and supported by PHN engagement teams.

Primary Sense not installed
If your practice uses Best Practice or Medical Director and Primary Sense is not installed, contact your local PHN for installation and training. Primary Sense is fully funded for general practices.
Using another clinical system
Most clinical systems provide reporting or data extraction tools that can help identify patients with key cardiovascular risk factors. Contact your local PHN or email phasesqld@c2coast.org.au to discuss the best approach for your practice.

Data and privacy
How patient data is protected
Primary Sense data is hosted on Primary Health Insights, a platform developed collaboratively by PHNs and hosted in Australia. Data is de identified and encrypted when extracted before it is transferred to the database. De identified data is stored in Microsoft Azure in Australia, supported by data governance and security controls designed to reduce the risk of unauthorised access.
Tools and integration
Accessing the AusCVD risk calculator
You can access the AusCVD risk calculator online.
Will AusCVD integrate with clinical systems
At present, the AusCVD calculator is not embedded within Best Practice or Medical Director. Practices can access it via a web browser.
Work is underway outside the PHASES project to enable deeper integration, including pre population of fields and returning results to the clinical record. Timing is not yet confirmed.
Does Primary Sense use AusCVD yet
Primary Sense currently uses Framingham based logic to estimate cardiovascular risk. Work is underway to update Primary Sense to align with AusCVD risk logic in the future.
Allied health
Role of allied health
Allied health plays an important role in supporting lifestyle changes and ongoing risk factor management. Allied health providers can strengthen collaboration by connecting with local general practices, clarifying shared care and referral pathways, and staying up to date with PHASES resources and local networks.
